This guide outlines how you can use form scoring in the WeGuide system for individual questionnaires, enhancing participant education and patient engagement.
You can use form scoring to implement evidence-based surveys such as PROMs and PREMs.

Different calculation types
Form scoring can be calculated for a set of questions in three specific ways that are listed below:
- Summation: The total score is calculated based on all questions’ responses.
- Mean: Mean is calculated as the total response score divided by the number of questions in the survey.
- Percentage: The formula for calculating the percentage is the total score divided by the sum of scores multiplied by 100. This basic percentage calculation is applied in the backend to derive the response percent of questions.
How Scoring works
Every individual question can be assigned weightage based on its significance in the overall survey.
Step 1: First, you need to toggle on the scoring checkbox for each question.
Step 2: You can add the corresponding score in the right-hand column for each response when adding a question.

Setup your form graph
After enabling form scoring on your questionnaire, you can set the minimum and maximum values for the Y-axis of your form. If you choose not to set these values, the Y-axis will adjust dynamically, using the lowest and highest values submitted by participants as its minimum and maximum.
Example of a score which has the minimum set to -40 and maximum to 60.
View results
- Once respondents complete all assigned surveys, an Admin or Clinician can check their responses from the Participant’s dashboard. Here is an example:
Limitations
- This feature only works with limited question types, namely Single choice, Multiple choice, Slider questions, and Number question types.
- It does not apply to Video questions, Photo questions & Text questions.
