In this article, we will explain how to create grouped questions and add them to your designated question groups. This allows users to view multiple questions on their screen simultaneously instead of just one at a time.
This approach is particularly useful when you need to capture multiple data points on a single screen, such as an address where fields like Street Name, Suburb, and Municipality can be displayed together.
How to group questions inside a form
Typically, questions are presented one at a time to participants. However, grouping questions can enhance user experience significantly. By presenting related questions on a single page, especially in lengthy forms or similar question sets (like addresses), you streamline the data collection process.

Step 1: Creating Question Groups
- On your unpublished health form, click on the Question Group tab and select + Add Question Group.
- Assign a name to your question group for easy identification later. Enter the name and click Save.
- Your question group is now created! Return to the Questions tab to start adding questions into your form group.
Step 2: Add Questions to a Question Group
- When adding a new question, you can choose whether it belongs to an existing group. Select the appropriate question group from the dropdown menu and save your form. The dropdown appears in the bottom right corner of the screen.
- You can easily remove a question from its group by selecting the blank option in the dropdown.
- Please note that some question types cannot be added to a question group due to their need for a single-screen interface (e.g., video questions). The dropdown for question groups will not be visible for these types.
Important: Questions within a group must be adjacent. You cannot group questions that are not directly next to each other. Ensure that any new question you want to add follows immediately after an existing grouped question; otherwise, you may encounter an error when saving your health form.
